In addition to the forms above, proof of the following vaccines by a licensed veterinarian are required:
- Current Rabies Vaccination Certificate (Dogs and Cats)
- Bordetella Vaccine within the last twelve months (Dogs)
- Pawadise recommends Bordetella every six months (Dogs)
- Pawadise recommends Bordetella every six months (Dogs)
- Current DAPP/Parvo Vaccine (Dogs)
- Current Distemper Vaccine (Cats)
- Highly Recommended - Current K9 Influenza (Dogs)
Your veterinarian may fax proof of vaccination to us prior to your arrival at 910-353-4849.
